Habilitation Specialist 1 (Qualified Intellectual Disability Professional*)
POSITION DESCRIPTION:
This Habilitation Specialist 1 (HS1) will be responsible for, completing assessments, facilitating recreation activities and other required documentation important for billing, and ensuring adherence to regulatory requirements for a both IRAs and community Intermediate Care Facilities (ICF's) which fall under the 483 regulations.
Primary Duties and Responsibilities
The Habilitation Specialist 1's duties will include, but are not limited to:
o Complete annual assessments for approximately 20 ICF individuals and identify strengths, needs, and interests for any ICF individuals o Monitor programs and revise as needed to ensure continual progress of individuals in reaching their goals o Communicate with treatment team members to gather additional information to plan appropriate services. Make modifications to services as recommended by the treatment team. o Provide assessment, training, and assistance to individuals to develop skills promoting independence in classroom and alternative settings o Record data and monthly summaries a timely manner and in accordance with agency standards to ensure they are complete and up to date. o Review monthly community outing data for 8 assigned residences and provide report to TTL. Assist with completing district-wide report of community outings and volunteer programs o Ensure objective or service plan data collection is completed for ICF individuals, as well as analyzing the data collected monthly and completing a monthly report for all objective plans. o Actively seek opportunities for individuals to develop relationships and participate in community life. o Organize and attend Special Olympics events for individuals to participate in track and field, bowling, softball, basketball, Bocce, and snowshoeing. Additionally, practice sessions and training for statewide Special Olympics events is required. o Participate in the Community Service Program through the Amenia Program to assist individuals with becoming involved in their local communities on a volunteer basis o Functions as liaison with the Self-Advocacy Committee and assists with scheduling and participation in Self-Advocacy monthly meetings. o Other duties as assigned by the TTL or Taconic leadership. Salary Range:
From $50715 to $64319 Annually
Minimum Qualification
Permanent/Provisional Appointments Available Posted Hiring Salary includes the sum of statutory hiring rate ($49,202) and Mid-Hudson Adjustment ($1,513). The Mid-Hudson Adjustment ($1,513) is also added to job rate ($62,806). Reachable on the Civil Service Eligible List for Habilitation Specialist 1 OR Have permanent service as a Habilitation Specialist 1 OR Eligible for 70.1 transfer to a Habilitation Specialist 1 title OR Eligible for provisional appointment pending examination. To be eligible for provisional appointment, candidates must possess a bachelor's degree or higher in a human services field. A degree in a human service field includes the professional disciplines of audiology, community mental health, nursing, nutrition, occupational therapy, physical therapy, psychology, rehabilitation counseling, vocational counseling, social work, speech/language pathology, and therapeutic recreation (including concentrations in art, creative arts, dance, drama, music, or physical education.) Also included are related academic disciplines associated with the study of human behavior (e.g., gerontology, human services, sociology, speech communication); or human skills development (e.g., counseling/guidance, education, human development.) Examples of non-qualifying degrees include but are not limited to:
criminal justice, political science, liberal arts, and business. If you are appointed provisionally, you must take the next scheduled civil service examination for this title and your score must be reachable on the resulting eligible list before you can be permanently appointed. If you do not pass the examination or achieve a reachable score, you may not be retained . OPERATIONAL PREFERENCE:
A Qualified Intellectual Disorder Professional - designation requires one year's experience working with people with a developmental disability AND a bachelor's degree or higher in a human services field (including, but not limited to:
sociology, special education, rehabilitation counseling or psychology).Estimated Salary: $20 to $28 per hour based on qualifications.
